Just fill in as much as you can, but PLEASE DO PROVIDE AN EMAIL ADDRESS. And please forward copies of the front and back of the insurance card(s) if you have it. We are always thrilled to speak with our clients and colleagues on the phone! However, in an attempt to streamline and simplify our intake process and serve as many clients as possible, as efficiently as possible, we do communicate via email often. Please inform patients of this when referring, and please ask them to include an email address where they can be reached promptly when filling out their paperwork. Thank you!
