Only REACH staff can refer families for an evaluation.
Families will provide insurance information.
You will be informed of evaluation costs for out-of-pocket and commercial insurance.
REACH will inform you of the testing date. Once confirmed, you will need to complete paperwork at least 2 weeks prior to the testing date .
We ask that you complete a release for us to contact your child’s teacher (if applicable).
You and your child’s teacher will receive behavioral measures via email to complete online BEFORE the testing date.
Attend the testing date with your child at the REACH (Northampton) office. You will receive feedback immediately following testing.
Approximately 4 weeks after testing you will receive a written report.
